Questionnaires

Open the questionnaire link you were given to start: enter your name there, answer each question, then submit at the end. Students with a Student ID can sign in to their dashboard below.

Your questionnaires are waiting on your dashboard. Sign in with your Student ID and login key to take the ones assigned to you and to see your past scores and answers.

How it works

  1. Sign in with your Student ID and login key (your administrator gives you these).
  2. Open a questionnaire from the To do list on your dashboard and press Start — no code to type.
  3. Answer the questions one at a time, then submit at the end. Your results appear under Completed.
